Outlier Health and Wellness offers competent medical care via telehealth and house call. We specialize in providing primary care for the elderly and chronic care homebound patients. For patients who have recently been discharged from a hospital, rehab, or nursing home, we offer transitional care visits. This service allows patients to have a medical provider care for them in their homes until they are stable enough to continue outpatient visits with their primary care provider. Patients who are stable and require follow-up for a chronic condition can have virtual/telehealth visits. These visits are also used for simple problem visits and our medical marijuana visits. Our certified providers offer certifications and prescriptions for medical marijuana to qualified eligible patients living in New York State.
